Milton Guerrero, MD, CMD

Milton Guerrero, MD, CMD practices as an attending physician in his own practice, providing direct patient care. He also serves as Medical Director to multiple nursing facilities around Houston and Huntsville, Texas. He has been working as a hospice and palliative medicine physician since 2002. His experience in long term care (nursing homes) started in 1997 and he enjoys tremendously what he does.

Dr. Guerrero graduated from the University of Cartagena, School of Medicine in Colombia in 1988. He completed his Family Medicine internship and residency at The University of Texas Medical School at Houston in 1994 where he also served as chief resident. He completed a fellowship program in Geriatric Medicine at Baylor College of Medicine in 1996. After completing his fellowship training, Dr. Guerrero became a full time geriatric professor with the Memorial Southwest Family Practice Residency Program, before moving into private practice.

Dr. Guerrero is Board Certified in Family Medicine and Geriatric Medicine. He was also Board Certified in Hospice and Palliative Medicine from 2007-2014. Dr. Guerrero developed an innovative program in Compassionate Care Hospice to care for patients with pulmonary disease. He is a Certified Medical Director (CMD) through the American Medical Directors Association (AMDA). AMDA is the professional association of over 5000 physicians who specialize in long-term care practice.

He is fluent in English and Spanish. His professional memberships include the American Academy of Family Physicians, The American Medical Directors Association, the American Academy of Hospice and Palliative Medicine, The Harris County Medical Society and the Texas Medical Association.

Lillian I. Flores-Perez is a licensed and certified nurse practitioner in the state of Texas. Lillian is bilingual fluent in both English and Spanish. She graduated from the University of Humacao, Puerto Rico in 1983 with a Bachelor Degree in Nursing where she became a Registered Nurse. In 1999, she graduated from the University of Texas in Houston with a Master Degree in Nursing and became Board Certified as an Adult and Geriatrics Nurse Practitioner by the American Nurse Credentialing Center and the American Academy of Nurse Practioners. Later in 2006, became Board Certified as a Wound Specialist by the Association for the Advancement of Wound Care. Most recently she became a Certified Hospice and Palliative Nurse.

For the past several years, Lillian has collaborated with physicians, patients, and families to provide excellent care for seniors in various settings, such as : clinics, skilled nursing facilities, long term care facilities, assisted living facilities, homes, and hospitals. Lillian has developed a NP/MD model in Long Term Care (LTC) and serves as a valuable resource and educator at LTC facilities.

In 2010, Lillian Florez-Perez was awarded the Bronze medal for Excellence in Nursing by the Good Samaritan Organization. For the past two years Lillian serves as the secretary of the Gulf Coast Geriatric Association of Nurse Practioners (GCGANP).

Abiola Williams (Abby), RN, BSN, MSN, GNP-BC

Abiola Williams (Abby) is a board certified Geriatric Nurse Practitioner. She had her first degree in Business Administration with a Masters in Business Administration in 1996 and 1999, respectively, but changing career was not a difficult option. Abby has a heart for the sick and a desire to make a difference.

She started her nursing career by attending Houston Community College where she graduated with an Associate Degree in Nursing in 2004. After obtaining this degree she was more certain that nursing is what she has always been called to be. She decided to attend Texas Tech University in Lubbock where she graduated with her Bachelor of Science in Nursing in 2006. She attended the University of Texas in Houston for her Masters degree in Nursing and graduated in 2009. She is board certified as a Geriatric Nurse Practitioner and is currently working on her PhD in Nursing.

For the last few years as a nurse, Abby has been involved in the care of adults and the elderly in the renowned Texas Medical Center providing passionate and excellent medical care in some of the best hospitals in the world and in nursing/assisted living facilities.

She does not see nursing as a career but as a ministry. She is grateful daily and one of the greatest is the opportunity to be there for one elderly person after another and their families. For Abby, taking care of this population is not just a job, but a way of life. Abby holds dear to her heart this cherished opportunity to be entrusted with someone’s life or the life of a loved one. Abby is married and has two beautiful children. Abby also serves as the web master for the Gulf Coast Geriatric Association of Nurse Practioners (GCGANP).
